Botox Aftercare: What to Do and Avoid in the First 24 Hours

You just left your Botox appointment, now what? The first 24 hours matter more than most people realize. What you do (and don’t do) in this window can influence how evenly your results settle and how long they last. Botox itself is a quick, minimally invasive treatment. The good news is that aftercare isn’t complicated. There’s no downtime, and you don’t need to overthink it. But following a few simple guidelines can help you get the smoothest, longest-lasting result from your treatment. Here’s a clear, hour-by-hour breakdown of what to do and avoid after Botox.

The First 4 Hours: Stay Upright and Hands Off

The first few hours after your Botox appointment are the most important. This is when the neurotoxin is settling into the targeted muscles, and anything that could shift the product needs to be avoided.

1. Stay Upright

Avoid lying flat, bending forward for extended periods, or napping face-down for at least 4 hours after your treatment. Lying down too soon can allow the product to migrate away from its intended placement. If you’re tired, rest in a reclined seated position or prop yourself up with pillows instead.

2. Don’t Touch, Rub, or Massage the Treated Area

Keep your hands off your face. Rubbing, pressing, or massaging the injection sites can move the product into muscles it wasn’t meant to reach, which can lead to uneven results or unwanted effects like a drooping eyelid or brow. If you need to clean the area, gently pat with clean fingers using a light touch. Skip facials, facial massages, and skincare tools like jade rollers for at least 24 hours.

3. Skip Makeup for a Few Hours

The injection sites typically close within about 15 minutes, but it’s a good idea to wait a few hours before applying makeup. When you do, apply it gently using light dabbing motions rather than rubbing.

The First 24 Hours: Take It Easy

Once you’re past the initial 4-hour window, you can return to most of your normal routine. But a few activities are still worth avoiding for the full first day.

4. Avoid Intense Exercise

Skip high-intensity workouts, hot yoga, and heavy lifting for at least 24 hours. Vigorous exercise raises your heart rate and blood flow, which can spread the neurotoxin from where it was placed. It also increases sweating, which can irritate freshly injected skin. Light walking and gentle stretching are fine after the first 4 hours.

5. Skip Alcohol

Alcohol thins the blood, which raises your risk of bruising and swelling. Avoid alcohol for at least 24 hours after your Botox appointment. Ideally, skip it for 24 hours before your appointment too.

6. Avoid Blood-Thinning Medications

Nonsteroidal anti-inflammatories like aspirin, ibuprofen, and naproxen can also increase your risk of bruising. If you regularly take these medications, ask your provider before your appointment about whether you should pause them briefly.

7. Sleep on Your Back

For the first night, try to sleep on your back to avoid pressing the treated area into your pillow. If you’re a stomach or side sleeper, this might take some effort, but it helps prevent uneven pressure that could shift the product. A silk or satin pillowcase can also reduce friction if you tend to move in your sleep.

The First 48 Hours: Watch the Heat

For the first two days, one more thing to keep in mind: heat exposure.

8. Avoid Saunas, Steam Rooms, and Hot Tubs

Heat dilates blood vessels and increases circulation, both of which can cause the neurotoxin to migrate before it fully settles. Skip saunas, steam rooms, hot tubs, and tanning beds for at least 48 hours.

9. Limit Direct Sun and Prolonged Heat

Avoid direct sun exposure and extreme heat for the first two days. If you’re outside, wear a broad-spectrum SPF 30 or higher, and consider a wide-brimmed hat for extra protection.

What You Can Do After Botox

Not everything after Botox is a restriction. Most of what you do day-to-day is fine right away. Here’s the short list of what’s okay:

  • Gently wash your face with lukewarm water and a mild cleanser
  • Return to work, drive, and go about your normal day
  • Drink water and eat normally
  • Have coffee or tea
  • Smile, laugh, and use your normal facial expressions, this won’t affect your results
  • Apply makeup gently after a few hours (light dabbing motions)
  • Take a lukewarm shower
  • Go for a light walk after the first 4 hours

When Will You See Results?

Botox doesn’t work instantly. Most people begin to notice results within 3 to 5 days, with full effects visible around 10 to 14 days as the neurotoxin fully settles into the muscle. If you’re still seeing full muscle movement or feel like your results aren’t quite where you expected two weeks after treatment, schedule a follow-up with your provider. Small adjustments may be needed, and that’s completely normal.

What’s Normal vs. When to Call Your Provider

A few mild side effects are common after Botox and typically resolve on their own within a day or two.

Normal Side Effects

  • Small pinpoint marks at the injection sites (fade within an hour or two)
  • Mild redness or warmth in the treated area
  • Minor bruising, especially around the eyes
  • A slight headache within the first day
  • Mild swelling that resolves within 24 hours

When to Contact Your Provider

Serious side effects are rare, but reach out to your provider or seek medical care if you experience:

  • Vision changes or blurred vision
  • Drooping of the eyelid or brow that doesn’t improve within a few days
  • Difficulty swallowing or speaking
  • Muscle weakness away from the treated area
  • Signs of infection at an injection site (increasing redness, warmth, or discharge)

These are uncommon, but worth knowing about, especially so you can spot them quickly if they happen.

How to Get the Most From Your Botox Results

Beyond the first 24 to 48 hours, a few longer-term habits can help extend how long your results last:

  • Wear broad-spectrum SPF daily, sun damage breaks down collagen and worsens lines
  • Stick with your normal skincare routine (retinols, vitamin C, and moisturizers are all fine after the first day or two)
  • Stay consistent with follow-up appointments before results fully wear off
  • Consider combining Botox with dermal filler, laser treatments, or professional facials for more comprehensive results
